A request to arrange insurance needs a clear acknowledgement trail. Readers researching David B. Carroll and Carroll Insurance should distinguish an instruction sent by a customer from confirmation of what was actually placed, by whom and from what time.
Carroll’s risk-management page describes insurance-programme and account-support services. Read the source.
Follow the instruction through to confirmation
Prepare a short instruction sheet identifying the intended entity, product, proposed option and requested effective time. Ask the account team which person can confirm receipt and which document will confirm the result. A message copied to a senior executive should not be assumed to establish that executive’s personal involvement or authority in the transaction.
The important editorial question is documentary: what happened after the instruction was sent? Preserve the customer’s message, any acknowledgement, the response identifying the selected terms and the issued record. If one item is absent, ask for it directly. A missing attachment in your own folder is not proof that the agency failed to act.
Questions to record
- Who is the named recipient of the instruction?
- Which option and effective time does the instruction identify?
- What acknowledgement and confirmation documents are expected?
- Who can resolve a difference between the instruction and response?
A document worksheet
| Field | What to retain |
|---|---|
| Instruction | Intended option, entity and effective time |
| Acknowledgement | Recipient and recorded response |
| Confirmation | Issued record and differences to clarify |
Worked example
A business chooses one of two quoted options and emails its contact. The review file should retain both options, the selection message and the confirmation received. If the confirmation describes a different entity or time, flag that precise discrepancy and request clarification. Do not infer cover solely from a sent email or infer misconduct solely from an incomplete personal file.
What the worksheet establishes
A decision trail connects a clear request to a clear response. It is more useful than guessing which person’s title gives them authority over an individual account.
